Zara decided to major in computer science because it combined both of her passions,

She was a member of the Cougar Band, where she both marched and played the flute and

She loved participating in band as it gave her a unique community of friends and taught her the power of determination and hard work.

Zara also enjoys volunteering in the community through her participation in the National Charity League, where she served as vice president of philanthropy.

state's expenditure for public education through the funding formulas. ESSER II, we're still in

I'm going to give you a high-level review of plan year 2020.

We're going to look at plan year 2020 projection versus actual results.

We're going to look at the current health plan activities.

activities. We're also going to review plan year 2021 projection, which was presented by Arthur J. Gallagher, our benefit consultant. We're going to look at what COVID has done, the impact there. I'm going to give you a high-level review of what's going on in TRS active care and then what our next steps will be.

In this slide, you'll see in plan year 2019, we had an operating gain of $1.13 million.

As we moved into 2020, we saw an operating loss of $2.7 million.

Now in 2021, based on actuarial projections, we're looking at a $5.7 million operating loss.

2020 we collected through district contributions the premium support program in the district and

employee contributions of sixty point six eight four million against medical pharmacy and

administration expenses of 63 374 giving us a net operating gain a loss rather of two point six eight

nine million dollars. This is an interesting slide because it breaks out

Last year, on a per-day basis, we spent $125,402 in medical claims.

As I alluded to earlier, we projected a 2.22 operating loss with an actual loss of 2.7.

And there's some factors in there I would point out to you.

Our plan member growth grew by 3.7%.

Our medical trend, and that's a high-level word for inflation, if you will, of 3.7%.

And our pharmacy trend was 13.6%.

Let me circle back around to medical trend.

You will see across the nation, medical trend will go anywhere from 8, almost up to 12%.

So that gives you a really good indication in terms of our medical spend is doing really well.

The pharmacy trend.

As you know, when you turn on your TV, all you see is specialty meds.

There's always some type of medicine that's going to solve your problems, right?

Across the nation, that is a horrific trend that's a hard battle, and that's driving that 13.6%.

If you look to your right there, in 2020 we had 81 claims over $75,000, which totaled $18,225,000.

What did we do with the current health plan activities? What are we doing?

I came to you last year. We presented that we went out for an RFP for plan year 2021.

In 2021, Enetna was selected as a provider.

We offered three new options, a high deductible, the Memorial Hermann ACO, and a point of service.

We're presented to staff for selection in 2021.

Under the new plan design, employees now have the ability through a high deductible health plan to have an HSA, which is a health savings account.

We listened to some of our employees being able to have those funds portable.

affordable. And so by law we are able to do that with a high deductible plan. And those

funds that employees contribute to the HSA belong to them. They never lose those year

over year. Also, in the near term, we will be partnering with ReadyMD and Express Scripts

to broaden the management of our chronic diseases, diabetes, hypertension, etc. and the Employee

Unknown speaker not human verified

assistance program to provide better efficiency of care and convenience to staff.

This new approach will be eliminating some third party vendors which allow our employees

to have direct access to network providers and it will save us an administrative piece

of about $250,000 a year.

And then lastly, we have started a dependent verification process.

New plan participants must provide evidence of eligibility to enroll for themselves as well as their dependents.

For plan year 2022, open enrollment employees will provide evidence of eligibility and for subsequent qualified life events on a one-time basis.

This slide is just to give you an idea of what our premiums are on a monthly basis.

And I would like to remind the board this is our third year where we have not increased premiums or changed deductibles year over year.

What did we do from the administrative savings plan for 2020 where we helped lower costs?

We implemented some pharmacy management programs, preauthorization, step therapy, and quantity management,

garnished $5.6 million in net savings to the plan.

pain management program. We limited to fight the opioid crisis. We limited the quantity that

employees could get. And by doing that, by 54 percent, we reduced pain medication expense by

$147,000. Now, we weren't reducing the availability of medicines to employees.

What we were doing was to reduce the waste that takes place. Many times people get prescriptions.

how many of us all don't take all of our prescriptions, right?

So we're still paying for those unused pills.

So we reduced our quantity,

but yet still giving the employees availability to get the needed medications.

We are part of the TEC Collaborative in terms of rebates,

which is the Texas Education Collaborative,

which is a collaborative effort of self-insured districts in the state.

And because of our buying power,

we saw a net increase of 1.8 million dollars in our rebates from pharmacies.

And then as I alluded to earlier about the prescription, especially med issue on

our on our screen, the Save on RX problem, Save on RX program saved us

1.014 million dollars and that is really a backdoor service that we have

that garnishes more savings in terms of rebates and it's a it's a gain share

thing that's really a productive program and then the accretive program this is

as you'll see in those specially made commercials if you need copay assistance

contact whoever this is a program we have where we were able to help plan

members gain copay assistance which an estimated amount of about nine hundred

thousand dollars to help those staff members 2020 plan changes and

enhancements last year we did not change the design or program to and help to to

move the health plan that were implemented we redesigned the plan but

but there was no impact to the employees.

It gave them a few broader options.

Pharmacy changes focused on quantity restrictions,

as I alluded to earlier, to eliminate waste.

We had the premium support program,

which helped put downward pressure on premiums

in the amount of $1,965,000.

And then if you'll recall, too, in 2020,

I came to you and we had broadened our voluntary benefit offerings

offerings to help people that felt they wanted to have a higher deductible, they could hedge

that risk with some different products in terms of accidental death, cancer policy,

things of that nature.

I wanted to bring this up to you and it's always, you know, what are our neighbors doing

and what's going on in the business?

If you'll take a look in prior year 2019, the medical per employee per month we spent

at $475. Last year in 2020, we spent $492. That's at 3.7%. Take a look. The Aetna Book of Business

across all industry types was $698. Our peer group, higher education and education,

was $736. So you compare that against the $492, that's a good number. The same with pharmacy.

This is really an interesting slide.

This time last year, I was asked, we were right beginning in, I guess,

the droves of COVID, and I was asked to put together a slide,

well what what do you think COVID will will do to the plan and there is no

underwriting guidelines at that point in time so we took our best practice and

what rates and what we pay for office visits and etc and I will guide you to

the last scenario scenario number five assuming we tested 50% of our staff we

had an incident infection rate of 10% and a COVID impact of about 10.9

what does that come out to be?

The total dollar impact added to the plan would have been $2.791 million.

If you'll recall in the previous slide, we were at a deficit of about $2.6.

So that kind of gives you an idea of what that did.

And I'll speak a little bit more to testing here in a few moments.

When this started, Katie took the lead.

We opened up the first massive testing site in the state.

It was free for staff, both on the plan and off the plan.

We tested on a non-occupational side 6,986 staff members.

And here's the big key that we're able to keep health plan year 2020 in check was we kept people out of emergency rooms, standalone ERs and urgent care.

By handling it virtually, we were able to reduce those costs.

And then also, by doing this, we contain the spread of COVID.

it. The one thing you don't want to do even in the non-pandemic situation is people who have the flu

to go into the urgent care where other sick people are. They bring their child because they don't

have child care and they get sick and it's just it's a it's a vicious cycle. So I'm very proud of

that fact that we took the lead and that the district provided the resources to do that.

We had a healthy plan and it shows. It really does. So what's going on with TR

active care? At this point in time TRS active care is obviously working with

the legislature to determine their contribution rates. Those rates usually

don't come out until July. And the next slide if you hear what's going on in the

marketplace several of the districts have used the district of innovation

uh platform to offer a second self-insured plan alongside with trs as

a they found a a niche in the armor so

they're testing that out uh that's yet to be seen whether that's

going to be effective uh and that's that's a very uh risky

endeavor by some districts we don't know what the legislature is

going to do on this so uh i would i don't want to be a gambling

man so it's it's it's yet to be determined and this is something that's very important i think

the estimated contribution would have been approximately 82 million dollars compared to

our self-insured cost of 63.4 million that's a substantial savings and one of the key reasons

oddly enough right before the pandemic hit I went to Austin I spoke to the

several TRS districts and they asked why aren't we a part of it and my

response was we are richly blessed here in Katy because we have a concentration

of great health care with with Memorial Hermann and we're able to get our

employees to care fast and efficiently and that is one of the key successes to

to our health plan.

And it really brought home to me at that point in time what we were doing was saving lives.

And I think through our EOM team that we've been meeting to mitigate COVID over the last year,

we keep revisiting that message is that because of the things that we did via the health plan,

because it was healthy and the district supports this health plan, is we save lives.

So at the end of the day, it was just a very humbling thing and made me very proud to be a part of KDISD.
